They must fill out the form with their full name, contact number, email address, preferred delivery address, store name, store branch, purchase date (within the promo period), model name, and product serial number. They must also upload proof of purchase, any government-issued ID, and a photo of the product serial number (found at the back or bottom of the actual monitor). If the purchase was made under a company, an authorized representative must upload a company ID together with a signed authorization letter.

Customers will receive a confirmation within 24 to 48 hours upon submission, and can claim their free Herschel bag within 15 days of purchase. The freebie will be delivered via courier free of charge, and includes a warranty slip.
